Abstract: In Hong Kong, children and young people who are unable to receive adequate care due to family problems are arranged to stay in small group home. However, once they reach the age of 18 and are no longer able to receive care from their families, they are required to leave the residential facilities. However, when these young people experience sudden environmental changes, they need better preparation and support to mitigate the negative impact on their psychological and emotional well-being. Therefore, in our study, we invited 2 in-service or ex-social workers and conducted qualitative interviews with 6 young people who have reached the age of 18 and left the small group home to understand their transition experiences and their perspectives on support service. Finally, the researcher will discuss the research findings, draw conclusions and make recommendations, and discuss the limitations of the study. This aims to provide readers with a better understanding of the challenges faced by young people after leaving care services and to provide more effective support measures to promote their personal development and successful adaptation to society.
